How to Apply

1. You need to complete an online application

Use the how to apply button on the JMU Graduate School homepage.

  • Be prepared to do the following:
  • Upload your personal statement, respond to the following question
    • In an integrated personal statement, please describe your unique contributions & perspectives, as they relate to your;
      • Values
      • Experiences
      • Connection to others
    • Explain how they are congruent with your goal to become an occupational therapist
  • Upload your unofficial transcripts verifying undergraduate degree and all pre-requisite course work. If you are enrolled in courses, you must update the OT Program by submitting transcripts verifying fall grades and spring registration. Send to
  • Upload a current Resume
  • Complete the Prerequisite Course Checklist
  • Record a video answering the question, Why would like to attend JMU OT Program (required)

2. Request your GRE scores to be sent to the Graduate School using code 5392

3. Complete and return the following: Reference Forms (3)  and  JMU Observation Verification Form(s)  and email to subject line your name and form name OR mail to
